Treatment and Care Programme

The Treatment and Care Programme Priorities:

The NHS Shared Planning Guidance for 2017-2019 set out transformation funding for supporting improvement in the treatment and care of people with diabetes.  Around £44 million of transformation funding will be used to improve treatment and care for the 2.8 million adults and children diagnosed with Type 1 or Type 2 diabetes in 2017/18.

This will include:

  1. Increasing uptake of structured education
  2. Improving achievement of the NICE recommended treatment targets (HbA1c, blood pressure and cholesterol for adults, HbA1c only for children)
  3. Reducing the number of amputations by improving access to multi-disciplinary foot care teams
  1. Reducing lengths of hospital stays by improving access to specialist inpatient support.


The importance of improving outcomes for individuals with diabetes is reflected in the inclusion in the CCG Improvement and Assessment Framework (CCG IAF) of indicators covering:

  • Diabetes patients that have achieved all the NICE recommended treatment targets: Three (HbA1c (blood sugar), cholesterol and blood pressure) for adults and one (HbA1c) for children’
  • People with diabetes diagnosed less than a year who attend a structured education course

These have been selected because they are recognised evidence-based measures indicating whether patients with diabetes are being supported to successfully manage their condition.


Our Work:

The Y&H Clinical Network has, and continues to, provide support to both bidding organisations and commissioners involved in this programme by maintaining focus on quality improvement, outcomes and evidence. This includes:

  • Supporting primary care to participate in the new National Diabetes Audit (NDA)
  • Sharing quality improvement tools with bidding partnerships that will facilitate the identification of appropriate actions to improve outcomes.
  • Sharing learning across and between bidding partnerships implementing similar initiatives
  • Assisting bidding partnerships in planning their Treatment and Care project milestones and evaluation.
