West Yorkshire and Harrogate Health and Care Partnership (WYH HCP) is one of three national dementia pilot sites exploring what work can be done collectively across an ICS area to reduce non-essential hospital admissions for people living with dementia.
Advance Care Planning and Conversation Skills - Training Package for Carers working in Domiciliary Care and Care Home Settings during COVID
A package of resources has been developed for carers working within care at home services and care homes during the COVID pandemic. The aim of this package is to introduce Advance Care Planning (ACP) and provide support to help staff develop the skills and confidence to have conversations with clients and residents about their future wishes for care.
This training has been tested at two ‘live’ sessions with care home and domiciliary care staff working across West Yorkshire & Harrogate. Of the 52 attendees who completed a poll at the end of training, 48 (92%) strongly agreed / agreed that their knowledge of Advance Care Planning had increased and 47 (90%) strongly agreed / agreed that they felt more confident about having Advance Care Planning conversations.

These resources are based on a training package which prior to the COVID outbreak was delivered using face to face cascade training. As this model cannot be currently be used due to social distancing requirements, the package outlined below has been developed to provide interim support and training during the COVID pandemic. It can be accessed remotely and at a time to suit the learner.
