The work of the Yorkshire and the Humber Stroke Network, as supported by the Clinical Networks, has now come to a close and these webpages will no longer be updated. 

Early Supported Discharge (ESD)

Early supported discharge (ESD) services aim to allow stroke patients to return home from hospital earlier than usual and receive more rehabilitation in the familiar environment of their own home.  The services are provided by teams of therapists and nurses.

The Yorkshire and the Humber SCN have been working with all providers in the region to standardise procedures and ensure all patient care in the region is equitable.  The Stroke Team are also working with commissioners and providers where there are no ESD services in place to support the development and commissioning of new services.

Work undertaken to date includes:

  • A complete picture of what ESD looks like across the region and where there are gaps in services.
  • The development of the Regional ESD Network.
  • Regional Standardisation of SSNAP data entry for ESD.
  • Working closely with colleagues in the East Midlands on SSNAP standardisation.
